The Stroke2gether project focuses on the development and scaling of a digital transmural recovery pathway for individuals after a stroke. It is a stroke care pathway that combines healthcare, lifestyle, and mental support in one remote program. This project started in January 2021 and spans a duration of 2 years, funded through EFRO resources. Howest collaborates with several partners on this initiative, namely (1) healthcare neighborhood Triamant, (2) hospitals AZ Groeninge and OLV Waregem, (3) technical developer Awell Health, and (4) Dashplus, (5) In4Care, and (6) POM West Flanders.
